Change Personal Details
Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you.
Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).
If you have moved within our catchment area, please provide us with proof of your new address. This can be a utility bill or bank statement dated within the last 3 months, or your council tax bill dated within the last 12 months.
If you have moved beyond our catchment area then you will need to register with a new GP in that area. This is so that we can keep an optimal patient list size and ensure we continue to provide safe, responsive and effective care to all of our registered patients. Thank you for your understanding.
