Please note that from 17 May 2021, your COVID-19 vaccination status can be accessed automatically through the NHS App without the need for Brockwell Park Surgery to grant you online access to your GP record.

If you do not have a smartphone and need proof of your COVID-19 vaccination status, please call the NHS helpline on 119 from 17 May and ask for a letter to be posted to you. In line with national guidance, GPs are unable to provide letters about your Covid-19 vaccination status.
