If you need a letter from your GP please email your request to Please include your name, date of birth, first line of your address, and details about the letter you need. We aim to process all requests within five working days. If you need to book an appointment to discuss the letter you will be notified by email.

Maternity exemption (MATB1) – we also need to know your due date.

Fit to fly letters – tell us your flight dates, due date (if pregnant) and confirm any medical conditions that you have.  There is a charge of £30.00 for this letter.

Fit notes (sick notes) – please include the reason you are unable to work, and date you are off work from and until. Please provide as much detail as you can. A doctor will call you if they need to clarify anything or discuss in more detail. If you are off work for less than 7 calendar days please self-certify.

Private referrals. Please email full details of your problem. This will be reviewed by one of our doctors. In some cases you will be asked to make an appointment to discuss further, or receive a telephone call.
